Care Administrator

ABOUT THE ORGANIZATION

2C-Health is a technology-driven organization striving to implement innovative solutions to human health care delivery. Our focus is on population health, improving the quality of life for the aging population and judicious use of resources.

SUMMARY

We are currently looking for qualified healthcare professionals for the role of Care Administrators who are passionate about remotely delivering care to the elderly and educating clinical staff and care coordinators. The ideal candidate should have an interest in learning new methodologies in team development and compliance adherence.

DUTIES AND RESPONSIBILITIES

The candidate is expected but not limited to perform the following tasks:

• Collaborate in developing a curriculum for clinical coordinators to deliver protocol-driven care (basic material will be provided).
• Gathering the most important information from the provided material or other peer-reviewed resources and formulate presentation material like powerpoint slides on a provided background template.
• Deliver online recorded and live course materials
• Responsible for monitoring and acting upon patient's chronic care management parameters.
• Chart review for compliance and documentation and guide care coordinators to improve on their professional duties
• Responding to any concerns from care coordinators

QUALIFICATIONS

EDUCATION REQUIRED AND/OR PREFERRED:

Required:

• Healthcare professional (Preffered)
• Bachelors in any Health Sciences field
• Diabetes Educator

LICENSURE/CERTIFICATION:

Preferred:

• Experience in providing healthcare services to patients.
• Nursing licensure

EXPERIENCE REQUIRED:

Required:

a) At least one year of relevant experience

KNOWLEDGE AND ABILITIES:

• A healthcare educationist must be proficient with English writing and speaking skills and must be technologically competent.
• Enthusiasm and passion for learning and taking responsibility are the key, guidance and training will be provided.
